- Parent Off-site Form (to be used in the event of a Parent Absence, up to 3 times per school year)
- Medical Authorization Form (Sample. In the event of a Parent Absence, it is recommended that you provide a Medical Authorization Form to your children's designated temporary caregiver. Please do not turn in to Legacy.)
